Shannon Pergament, MPH, MSW, is a founding member of the Somali, Latino and Hmong Partnership for Health and Wellness (SoLaHmo), a community driven translational research program of Community University Health Care Center (CUHCC), a federally qualified health care center in Minneapolis, MN.
During the last 11 years, Pergament was Co-Director of Community Based Research at Minnesota Community Care where SoLaHmo was founded. She holds Master of Public Health (M.P.H.) degree and Social Work (M.S.W.) degrees from the University of Minnesota. For over 25 years, Shannon has worked in the area of health equity, building collaborations with ethnically diverse communities to promote community and cultural assets and reduce health disparities through community based participatory action research (CBPAR), health education and policy.
